The Ilizarov apparatus is a type of external fixation used in orthopedic surgery to lengthen or reshape limb bones; to treat complex and/or open bone fractures; and in cases of infected non-unions of bones that are not amenable with other techniques. It is named after the orthopedic surgeon Gavriil Abramovich Ilizarov from the Soviet Union, who pioneered the technique.

Ilizarov Small Bone Fixation System

The ILIZAROV Small Bone Fixation System can be used for small, pediatric and upper extremity applications. With its smaller half rings, wires and lightweight design, the system permits stable fixation of very small bones, bone fragments and osteoporotic bones. Simultaneous corrections of multiplane deformities are also achieved with the same precision as the original Ilizarov system. The ILIZAROV Small Bone Fixation System also features:

  • A wide variety of indications treatable with one system
  • Unique hole patterns that deliver the extra convenience of additional holes (with respect to normal Ilizarov half rings) without reducing stability
  • Half ring sizes from 40 mm to 120 mm
  • Small 1.0, 1.2 and 1.5 mm smooth and olive wires, which allow for stable placement in very small bones, fragments and osteoporotic bones

How is it different from conventional orthopaedic techniques?

No treatment method in the history of reconstructive traumatology and orthopedics has had such an evolutional development as the Ilizarov method of transosseous osteosynthesis. The method has been called universal as it can be used for almost entire range of skeletal pathology that requires reconstruction and bone reshaping. Nowadays, it has been used in associated fields of medicine such as angiology, neurosurgery, oncology, stomatology, arthrology, veterinary, etc.

The Italian orthpaedic surgeons called the method the Revolution in Orthopaedics and named G. Ilizarov Michelangelo of Orthopaedics.

It has been unanimously recognized that the method enables improve treatment outcomes in patients with bone tissue diseases in the whole world, and especially in cases of congenital pathology or trauma sequelae. For 55% of those who are affected by orthopaedic pathology it is the only method that can recover them or improve their condition. It is less traumatic as compared to other methods; and the intervention procedures are less in volume in case the patient needs a complex operation or repeated surgeries. There are a lot of Ilizarov techniques that are bloodless and do not require bone grafting. Patients can start walking with the fixator on their legs on the second day after the operation and have normal self-service and life activities during the entire period of treatment.

Can this technique used for height growth?

Yes, this is the first method used for limb lengthening and height growth. In the recent years, the number of people who refer to orthopaedic surgeons with a wish to change their height has considerably grown. Since the middle of 1970s, more than 500 persons increased their stature at the Kurgan center due to indications to a lengthening procedure, and they are mainly patients with achondroplasia (dwarfs).

In order to master the method, one should study, and the Ilizarov method training takes a lot of time and also can be costly for most residents as the best training can be obtained in Russia where the method was born. That is why, there is a constant interest in the method but its practical application is performed by those surgeons who have been well trained and have experience. This is true and should be so. The method gains good results only in skilled hands.

Another difficulty that the method encounters is the politics of the hospitals and medical authorities, as the Ilizarov treatment procedure, though being advanced and with efficient outcomes, takes a lot of time. The economic situations of public hospitals are not good, they usually cannot afford long inpatient stay, and rural patients cannot come often for outpatient treatment or cannot afford it at all… So, there is a lot of work for charities and governmental medical authorities for improving the situation so that the Ilizarov treatment could be available to a bigger number of patients.

Total Hip replacement is a surgical procedure in which the hip joint is replaced by a prosthetic implant. Hip replacement surgery can be performed as a total replacement or a hemi (half) replacement. Such joint replacement orthopaedic surgery is generally conducted to relieve arthritis pain or in some hip fractures.

Who Should Have Total Hip Replacement Surgery?

The most common reason for hip replacement is osteoarthritis in the hip joint. Your doctor might also suggest this surgery if you have:

  • Rheumatoid arthritis (a disease that causes joint pain, stiffness, and swelling)
  • Osteonecrosis (a disease that causes the bone in joints to die)
  • Injury of the hip joint
  • Bone tumors that break down the hip joint.

These treatments may decrease hip pain and improve function. Sometimes the pain remains and makes daily activities hard to do. In this case, your doctor may order an x ray to look at the damage to the joint. If the x ray shows damage and your hip joint hurts, you may need a hip replacement.

How Should I Prepare for Surgery?

To prepare for surgery, you can:

  • Learn what to expect before, during, and after surgery

  • Put things you need in one place at home (for instance, put the remote control, radio, telephone, medicine, tissues, and wastebasket next to your chair or bed)
  • Place items you use every day at arm level to avoid reaching up or bending down
  • Stock up on food
  • Make and freeze meals.

What Should I Do After Surgery?

Soon after surgery, you will meet a respiratory therapist and a physical therapist. The respiratory therapist may ask you to breathe deeply, cough, or blow into a device to check your lungs. Deep breathing helps to keep fluid out of your lungs after surgery.

The physical therapist will teach you how to sit up, bend over, and walk with your new hip. The therapist will also teach you simple exercises to help you get better. In some cases, within 1 to 2 days after surgery, you may be able to sit on the edge of the bed, stand, and even walk with help.

Knee replacement, or knee arthroplasty, is a surgical procedure to replace the weight-bearing surfaces of the knee joint to relieve pain and disability. It is most commonly performed for osteoarthritis, and also for other knee diseases such as rheumatoid arthritis and psoriatic arthritis.

Arthroplasty is a field of medicine which deals with the surgical reconstruction and total replacement of degenerated joints. Arthroplasty uses artificial body parts (prosthetics). Arthroplasty literally means “the surgical repair of a joint”.

When the articular cartilage of the knee becomes damaged or worn out, it becomes painful – the patient finds it extremely hard to move the knee. The bones, rather than sliding over each other with the minimum of friction, rub and crush together.

If an artificial prosthesis is surgically implanted, the patient will feel much less pain, possibly none, and his knee will move properly.

Replacement surgery in a damaged knee joint by placing an artificial prosthesis will alleviate pain and help better movement of the knee.

Today, every year, over 600,000 knee replacement surgeries are carried out in the United States and more than 70,000 in England and Wales. Most patients are seniors – aged over 65 years.

For most patients, a replacement knee surgical procedure will last for at least 15 to 20 years, especially if cared for properly and not put under too much strain. More than 90% of people who have total knee replacement surgery experience a dramatic decrease in knee pain and a significant improvement in their ability to perform common activities of daily living.

However, total knee replacement will not let you do more than you could before you developed, for example, arthritis.

Reasons to perform knee replacement surgery

Today, a knee replacement surgery is considered a routine operation. Below are the three most common reasons for the procedure:

  • Osteoarthritis: This type of arthritis is age related – caused by the normal long wear and tear of the knee joint. The majority of patients are over 50; however, younger people may be affected. This is a type of arthritis caused by inflammation, breakdown, and the gradual and eventual loss of cartilage in the joints – over time, the cartilage wears down. When the bones rub against each other for a few years, they may compensate by growing thicker, but this will result in more friction and more pain.
  • Rheumatoid arthritis: It is also called inflammatory arthritis, occurs when the membrane surrounding the knee joint is inflamated and thick. This inflammation becomes chronic and will damage the cartilage causing soreness and stiffness.
  • Post-traumatic arthritis: This type of arthritis is due to a severe knee injury. When the bones around the knee break or the ligaments tear, this will affect the knee cartilage. Sometimes, surgery is the best option.

When is knee replacement surgery recommended?

Knee surgery is recommended for older people, although adults or teenagers of any age can be candidates for the procedure, since they are physically active and will more rapidly wear the joint out. The weight, gender, or age of the person is never a factor when considering knee replacement surgery. Whether or not to perform surgery is nearly always based on the severity of pain and degree of disability of the patient.

If should be noted that knee replacement surgery that occurs earlier in life usually means further surgery later on. However, several studies have proven that knee replacement surgery performed before severe stiffness and pain set in is associated with better outcomes.

When considering the option of knee replacement surgery, doctors take into account a number of symptoms:

  • Severe knee pain or stiffness: Do symptoms seriously undermine the patient’s ability to carry out everyday tasks and activities, such as walking, going upstairs, getting in and out of cars, getting up from a chair, etc?
  • Moderate but continuous knee pain: Is pain present while sleeping or resting?
  • Chronic knee inflammation and swelling: Does the swelling not improve after taking medications or resting? Do drugs cause unpleasant side effects?
  • Knee deformity: Is there is a noticeable arch in the inside or outside of the knee?
  • Nothing else worked: The doctor has prescribed medications and physical therapy without any substantial improvement.
  • Depression: Depression can be a serious, debilitating and devastating illness. Chronic pain and problems with mobility can eventually lead to depression, especially if the patient cannot do normal daily or social activities properly. Replacement knee surgery may prevent depression from occurring, or help get rid of it.
